PHQ-9 (Patient Health Questionnaire-9) - Depression

The PHQ-9 is a nine-item depression scale that is based on the diagnostic criteria for major depressive disorder in the DSM-IV. The PHQ-9 can be used as a screening tool, an aid in diagnosis, and as a symptom tracking tool. The nine items of the PHQ-9 cover different aspects of depression, including how often you have been bothered by depressed mood or loss of interest in activities, how often you have been bothered by trouble sleeping or tiredness, and how often you have been bothered by poor appetite or weight loss. Each item is rated on a scale from 0 to 3, with 0 being "not at all" and 3 being "nearly every day." The total score can range from 0 to 27, with higher scores indicating more severe depression. The PHQ-9 can help to track the severity of depression symptoms over time and can also be used to monitor the response to treatment.
